Learn all about care and nursing management of older persons with multiple co-morbidities and healthcare needs.

Learning Outcomes

You will acquire the knowledge and skills in health assessment and clinical decision, applied pathophysiology and pharmacology, principles and concepts of community gerontology nursing, primary home healthcare and palliative care nursing._x000D_

The course is suitable for in-service registered nurses with valid practicing certificate from the Singapore Nursing Board (SNB), and preferably with practice in the care of older adults in the intermediate and long term care settings, primary healthcare settings and community-based hospitals.

Module
Health Assessment and Clinical Decision Making
Provides foundation and principles of health assessment, comprehensive skills necessary to obtain data through complete health history taking and physical examination.

Applied Pathophysiology and Pharmacology
Introduces knowledge and understanding of pathophysiologic and pharmacologic considerations in gerontology.
Post-Diploma Certificate in Community Gerontology Nursing 2
Module
Principles and Concepts of Community Gerontology Nursing
Provides the essential knowledge focus, framework, tools and skills the nurse needs to know when caring for the older adults.
Module
Primary, Home Healthcare and Palliative Care Nursing
Addresses the major primary health care conditions commonly seen by the community gerontology nurse in the primary health care clinic.
Module
Clinical Practicum in Community Gerontology Nursing
Enables consolidation of clinical care and skills from discharge planning at acute or community hospitals to supporting service organizations. Evaluates community based services and gain experience in social assessment, care planning of the community dwelling elders and the step down community services.
Module
On-the-Job Training (OJT)
This OJT-based module allows students to integrate the skills and knowledge acquired from the course according to the OJT blueprint checklist designed by the workplace in accordance to the SkillsFuture Earn and Learn framework. A workplace mentor will be assigned to facilitate the learning. A pass/fail grade will accorded for its assessment conducted by the workplace mentors in consultation with NP.

You are required to complete 2 post-diploma certificates within 2-year validity period to be awarded the Specialist Diploma qualification.


Entry Requirements

Applicants with any of the following qualifications are invited to apply for the course:

  • Registered nurses with valid practising certificate from Singapore Nursing Board (SNB)

  • A local Polytechnic Diploma / Degree in Health Sciences (Nursing) or equivalent
  • _x000D_

Preference will be given to those who have worked with elderly patients._x000D_

Successful applicants will need to undergo a compulsory medical examination and vaccinations before semester starts. Please see details and instruction in the medical examination form for submission of all the required documents.
